Group Medical Insurance: Bluegrass Family Health insurance is available to full-time employees (.8 FTE or greater). Employees become eligible for coverage the first of the month following 60 days of eligible employment. The following are brief descriptions of coverage and employee cost.

Vision coverage is included as part of the Bluegrass Family Health insurance with a office visit co-pay at a participating provider and a $100 vision hardware maximum. Delta Dental also has a discount vision plan with one of their participating providers.
Group Dental Insurance- Delta Dental insurance is available to all full-time employees (.8 FTE or greater). Employees and dependents become eligible the first of the month after 60 days of eligible employment. Employees must go to a participating dentist. This plan has a maximum benefit of $1000/period and pays 100% of Diagnostic & Preventive services (not subject to deductible; 80% of Minor services and 50% of Major services after a $25 individual/$75 family deductible. Employee costs per pay period are: Employee only $7.24; Employee + One person $15.92; Employee + Family $24.67.
Group Life Insurance- Life insurance is offered through Humana and is provided to all full-time employees (.8 FTE or greater) at no cost to the employee. Benefits include: Employee benefit 1 ½ x annual salary up to a maximum of $50,000. Coverage is also provided for the employees spouse in the amount of $5000 and $2500 for dependents up to the age of 25 if a full-time student. Eligibility begins 60 days from the date of eligible full-time employment.
Pre-Tax 125 Plan- The employee has the option to have benefits deducted from their paycheck pre-taxed. This decreases the employees taxable income. Note: once an employee has elected the Pre-tax 125 and enroll into benefit(s), the employee is locked in this program for one year and cannot discontinue or change the option or benefit(s) unless there is a qualifying event.
Retirement Plan (KY Deferred Compensation)- Employees may begin contributing into a 401(k), 457, Roth 401(k) or Roth IRA through the Kentucky Deferred Compensation Group immediately upon obtaining part-time or full-time status (.4 FTE or greater). FCH will begin making their contribution into the fund when the employee has been eligible and employed for six months prior to contribution. FCH currently provides 3% of the employees’ gross annual income. 100% vesting begins immediately upon FCH contribution. Effective 01/01/08, employees who are hired or become eligible for benefits on or after January 1, 2008, must contribute a minimum of $15 per pay period in order to be eligible to receive employer matching of 3%. Employees hired or who become eligible for benefits on or after January 1, 2008 will be vested 20% per year and at the end of the 6th year will be 100% vested.
Paid Time Off (PTO)- is a flexible program made up of two major components: Occasional Time Off (OTO) and Extended Time Off (ETO). OTO combines vacation and holiday as well as sick days for occasional short-term illness. ETO is a bank of time intended to help reduce a staff member’s financial hardship during and extended illness. Employees accrue PTO each pay period based on their length of service and paid hours for the pay period. Employees are eligible for this benefit if they are a .4 FTE status or greater and may begin using OTO after they have successfully completed a 90 day Initial Employment Period. For more information and details regarding accrual schedules for salaried and non-salaried employees, please reference policy PER-007-17 in the Administrative manual.
Employee Assistance Program (EAP)- is available for full-time and part-time employees only (.4 FTE or greater). FCH recognizes the need for employees to deal with daily stressful situations whether at work or home setting. FCH will authorize eligible employees to obtain counseling services through an approved agency and will pay for up to four (4) visits per year. For more information regarding this program, please see the Human Resources Office.
Tuition Reimbursement Program- FCH offers tuition assistance program to those employees who have been regularly scheduled to work an average of at least 32 hours per week for a period of one calendar year prior to the anticipated start date of the course or as approved by Administration. The course(s) should be related to the employee’s hospital duties and responsibilities or be related to healthcare advancement for clinical and non-clinical staff. All requests will need approval from the CEO. Obtain additional information and apply for assistance through Administration.
Courtesy Discounts- All part-time and full-time employees and their immediate family (spouse, dependent children) are eligible for a 50% discount on balances paid after receipt of insurance payments, for services rendered at FCH, whether in-patient or out-patient services. Discounts through the hospital pharmacy are available for part-time and full-time employees and their immediate family. All employees will receive a 25% discount for meals obtained through the hospital cafeteria.
Cafeteria Insurance Plans- coverage for cancer, accident, critical illness, short-term disability, universal and term life as well as various other plans are available for purchase by full-time employees through Colonial Life Insurance Company. Payroll deductions are available. Employees will need to meet with the Colonial Life Insurance representative to obtain quotes for these types of services. Please advise Human Resources of your interest.
PHI Air Ambulance Membership Program- voluntary coverage through PHI provides medically necessary transport by of their aircraft. PHI bills your insurance and accepts what your insurance company pays as “payment in full”. Cost to the employee is per the Air Membership Application. A one-time payroll deduction/year is permitted.
